Code of Conduct- Needed for all 7th and 8th Graders for each season. 9th-12th graders it is included in your extra curricular activities in student handbook signed in the beginning of the year. Turn into Coach before first practice

Interval Health History- Needed for all student athletes each season. Turn into School Nurse before first practice

NYS School Health Examination Form - Only needed if you receive a physical from your private physician
